GLP-1 hair loss: how common it is, why it happens, and what helps

Last reviewed · By GLP1 Sidekick Editorial · 4 min read

The short answer

Hair loss is a listed side effect of Zepbound, Wegovy and Foundayo, reported by about 3–7% of people in trials versus 1–2% on placebo, and more often in women. The labels say it was associated with weight reduction: rapid weight loss is a known trigger of temporary shedding (telogen effluvium), which the American Academy of Dermatology says usually regrows to normal fullness within six to nine months. Eating enough protein and checking for other causes like low iron or thyroid problems are sensible steps.

Does Zepbound, Wegovy or Ozempic cause hair loss?

Hair loss appears in the FDA-approved labels:

MedicineHair loss on drugOn placebo
Zepbound 5 / 10 / 15 mg5% / 4% / 5% (7.1% of women vs 0.5% of men)1%
Wegovy injection 2.4 mg3.3% (4% of women, 0.9% of men)1%
Wegovy HD 7.2 mg5.8% (8.4% of women, 0.2% of men)1.0%
Foundayo 5.5 / 9 / 17.2 mg4% / 4% / 5%2%

The Zepbound, Wegovy and Foundayo labels all state that hair loss was associated with weight reduction. In Zepbound’s trials, no one taking the drug stopped treatment because of hair loss. Ozempic (for type 2 diabetes) lists alopecia among reactions reported after approval.

What did the 2026 BMJ study find?

A study in The BMJ, reported July 22, 2026, used Penn Medicine health records for adults with type 2 diabetes. People starting a GLP-1 had a higher risk of clinically recorded hair loss than those starting SGLT-2 inhibitors (hazard ratio 1.37) or DPP-4 inhibitors (1.68). Absolute rates were low—about 6.9 vs 5.0 cases per 1,000 person-years in one comparison. The increase was mainly non-scarring hair loss, the type typically triggered by rapid weight loss and usually reversible. The authors suggested hair loss may reflect the downstream effects of metabolic change rather than a direct drug effect.

Why does rapid weight loss cause hair shedding?

The American Academy of Dermatology says excessive shedding—called telogen effluvium—is common after stressors including losing 20 pounds or more, giving birth, illness or surgery. Most people notice it a few months after the trigger. It’s typically temporary: as the body readjusts, shedding stops, and hair tends to regain normal fullness within six to nine months. Eating much less can also lower intake of protein, iron, zinc and other nutrients; the 2025 joint advisory lists excessive hair loss among signs of possible nutrient deficiency.

What can I do about hair loss on a GLP-1?

  • Prioritize protein at each meal (see what to eat on a GLP-1).
  • Ask about lab work, such as iron and thyroid tests, to rule out other causes.
  • Ask whether a multivitamin makes sense for you—the advisory says supplements can be considered for at-risk nutrients; don’t start high-dose supplements on your own.
  • See a dermatologist if shedding is heavy, patchy, lasts more than several months, or comes with scalp symptoms. They can discuss treatments such as topical minoxidil.
  • Don’t stop your medicine on your own; talk with your prescriber about how fast you’re losing weight.

Is GLP-1 hair loss permanent?

Shedding linked to weight loss is usually temporary, according to the AAD and the BMJ authors. But hair loss has many causes, and the BMJ study also found a small signal for scarring hair loss compared with one drug class. If your hair isn’t recovering, get it checked rather than assuming it’s the medication.

When does weight-loss hair shedding start and stop?

StageWhat the AAD describes
TriggerA stressor such as losing 20 pounds or more
OnsetShedding usually noticed a few months after the trigger
RecoveryShedding stops as the body readjusts; hair tends to regain normal fullness within 6–9 months
If the stressor continuesShedding can last longer

The BMJ authors suggested clinicians watch for hair loss particularly in the first year of GLP-1 treatment.

Could something else be causing my hair loss?

Yes. The AAD lists other common triggers of excessive shedding, including giving birth, a lot of stress, a high fever, an operation, recovering from an illness, and stopping birth-control pills. Thinning that runs in families (pattern hair loss), thyroid problems and low iron are other causes a clinician may check. The 2025 joint advisory lists excessive hair loss among possible signs of nutrient deficiency, alongside unusual fatigue, muscle weakness and poor wound healing—worth mentioning to your prescriber if you notice several together.

Quick answers

How common is hair loss on Zepbound?

In Zepbound’s weight-management trials, hair loss was reported by 4–5% of people on the drug vs 1% on placebo. It was more frequent in women (7.1%) than men (0.5%), and the label says it was associated with weight reduction.

Does Ozempic hair loss grow back?

Shedding triggered by rapid weight loss (telogen effluvium) is usually temporary. The American Academy of Dermatology says hair tends to regain normal fullness within six to nine months once the trigger passes. See a doctor if it doesn’t.

Can protein or vitamins stop GLP-1 hair loss?

Getting enough protein and nutrients helps your body cope with weight loss, and low iron or other deficiencies can contribute to shedding. Ask your clinician about labs and whether a supplement makes sense; no supplement is proven to prevent GLP-1-related shedding.

Is hair loss more common on Zepbound or Wegovy?

The labels report similar low rates: 4–5% on Zepbound vs 1% on placebo, and 3.3% on Wegovy 2.4 mg vs 1% on placebo (5.8% on Wegovy HD 7.2 mg). These come from separate trials, so they can’t be compared directly. Both are higher in women.
